Your First Appointment

We’re excited that you’re thinking about making an appointment with Dr. Howard and taking the initiative in creating the beautiful smile of your dreams. To help you sort out the logistics of your visit, you can find everything you need to know before scheduling your first appointment on this page. Your First Visit

At your first visit, you’ll need to fill out some paperwork that will let us know about your overall health and any preexisting conditions we need to be aware of. You’ll also have a chance to tell Dr. Howard about any specific needs or concerns you have in person. Once we’ve taken the time to evaluate any current dental issues, we’ll perform a comprehensive dental and periodontal examination utilizing digital dental x-rays to help diagnose and see conditions not visible to normal eyesight. Often times we can also do a teeth cleaning at this same visit. We may schedule further procedures depending on the state of your oral health.

New Patient Forms

We recognize how valuable your time is, and we’d like to give you the chance to reduce as many as 15 minutes from your initial time on the day of your appointment. Instead of filling out your forms on the day of your appointment, you can download them using the link below. Once you’ve printed them out, you can take your time completing them at home so that when you arrive at our dental office, you can hand it over right away and speed up the overall process.

Dental Insurance

We accept most dental health insurance plans at both of our locations in Rancho Bernardo and San Marcos. We will file claims on your behalf. We ask that you be prepared to pay your co-pay and/or deductible at time of services rendered. Your insurance can be a big help in getting the care you need, and we will help to assure that you receive the maximum benefits possible. However, the contract between your employer and the insurance company will not affect your responsibility for full payment of any fees. If your insurance doesn’t pay what we expect, then you are
responsible for any balance.

How do you plan on paying for your dental care? Different methods of financing work best for different patients, and we are ready to accommodate you no matter what your situation is. Call us ahead of time to discuss your financial situation and ask any questions you might have about payment. We will be happy to accept multiple dental health insurances, including Delta Premier. Non-insured patients can look into qualifying for third-party financing through CareCredit.

While you do have the option of paying for your treatment right away at our dental office, we recognize that every patient has different needs. That’s why we also offer the opportunity to pay your dental bills through our website. You can check your balance and pay whenever you’re ready using PayPal. Your personal information will be kept completely secure, so you can feel confident about using this payment method.
